It begins at the outerlayer of the lung called the parietal pleura. Gradually this cancer spreads to the whole lung. It is a rare type of cancer, and thesourcesofitstreatmentare also limited. For this reason, itssuccessfultreatmentisusually unavailable or only in high centersofbigcities.Thereare many reasons for the occurrence of this cancer, in which asbestos,mining,andsilicaare the main ones. It is generally seenthattheeffectofthis type ofcancerismoreinDungarpur, Banswara. Whena37-year-oldpatient from Dungarpur was brought to the Geetanjali Cancer Center, he was first given three chemotherapy cycles. After this, the patient underwent surgery which is called extrapleuralpneumonectomy. Extrapleuralpneumonectomy is surgery to remove a diseased lung, including part of the pericardium (the membranecoveringtheheart),part of the diaphragm (the muscle between the lungs and the abdomen), and part of the parietalpleura(themembrane of the chest). They are removed simultaneously. A similar treatment for pleural mesothelioma cancer was used until a few years later. In today's context for pleural m e s o th e l i o m a c a n c e r, advanced techniques are called extrapleural pneumonectomy radical surgery combined with hyperthermic intrathoracic Chemotherapy (HITHOC). This patient was successfully treated with this state-ofthe-art technique. Under this, allthediseaseis removedduring surgery, but some microscopicdiseaseremains,which is not possible to see by the eyes; for that high-temperatu r e m a c h i n e c o m e s , Chemotherapy is given at a 42-degree temperature. Usually, this type of operation is expensive. It is done by keepingonlyonelungrunning, and the critical anesthesiologist has a significant role in it because this surgery takes 5- 6 hours by a single lung and h a s to b e m a n a g e d . | Chemotherapy is circulatedin the patient's chest for a dead hour at 42 degrees. In such a case,aheartattack canoccur. Apart from this, there can be side effects on the patient's body. A critical team is used for post-operative care. The patient has to be kept in ICU forabout2-3days,andallnecessarysupporthastobegiven. Afterthis,thepatientstartsgetting health benefits. This case of Extrapleural Pneumonectomy Radical Surgery Combined with Hyperthermic Intrathoracic Chemotherapy (HITHOC) is the first case in the entire Rajasthanthathasbeendone fr e e o f c o s t u n d e r Mukhyamantri Chiranjeevi Yojana. This type of technology isavailableonly inthehigh centers of big cities.
